Yes, changing your link structure will break links. But if you are going to change it then I recommend doing 2 things:
1. Change your custom permalink structure to /%postname%/ .
2. Use a plugin like one of these for Wordpress.
http://www.deanlee.cn/wordpress/permalinks-migr...
http://urbangiraffe.com/plugins/redirection/
They promise to change your permalink structure without breaking the old links to your website and not affect your search engine rankings either with a “301" Permanent Redirect.
I haven't tried any of these personally so use at your own risk.
If you don't like it you can revert back to your current permalink structure again.
I saw Lee's question about changing the permalinks. Just to let you know that with my hosting provider, I got troubles doing so.
Few weeks ago I changed the permalinks with Wordpress standard feature (1), and I got an error : file not found on all articles. Oops !
It was due to an automatically generated file, .htacess, located at the root of the site. And this file content was not compatible with my hosting system. I had to delete the file to keep the blog back on tracks (and with the pretty permalinks).
